Self-management Using Collaborative Care EnhancEment in Diseases

Web-SUCCEED Study logoWeb-SUCCEED is a self-management program for people living with chronic health conditions and their caregiver (family members or friends who provide care to them). It aims to help patients be as successful as possible with managing their chronic health condition by supporting both the patient and their caregiver. Chronic health conditions affect friends and family, especially those who might be caring for them. While the person living with a health condition(s) has to cope with the physical problems, the caregiver often takes on more responsibility around the house or for managing contacts with the health care team. For this reason, support for both members of the dyad is an important part of chronic health condition care.

Web-SUCCEED is based on programs that have helped people with health conditions do what they need to do to stay as well as possible. The Web-SUCCEED program is unique because it focuses on how patients and caregivers work together. Web-SUCCEED involves learning about chronic health condition management, how to manage stress or symptoms of depression, and practicing skills people need to communicate effectively with others and maintain a healthy relationship.

Why does the web-SUCCEED Study matter?

The web-SUCCEED Study is important because it will test a new, web-based program that has been developed based on theories. This program can positively impact the lives of Veterans with chronic health conditions and those who care for them.

Who can participate?

  • Veterans diagnosed with a chronic condition and receive help from a family member/friend AND a family member/friend who helped provide care
  • Have access to a computer with an internet connection

What will you do?

  • Share your experiences with managing a chronic condition
  • Total study duration: 6 months
  • Complete 3 surveys at 3 different time points [approx. 1 hr]
  • Participate in 4 weekly coaching sessions [approx. 1 hr] & complete online modules [approx. 30 min] if assigned to the Web-SUCCEED intervention
  • Receive up to $150 for your participation
  • Location: virtual, teleconference online
