Background/Rationale: Black-White disparities in control of hypertension and diabetes contribute to U.S. racial disparities in life expectancy. Within VA, higher rates of uncontrolled hypertension or diabetes have been identified in several racial/ethnic minority groups, compared with white Veterans.
